Hebrew for Kids Registration & Checklist:

Complete the online registration form below, one for each child.
Upon completion of the form, you will be prompted to a payment form:
- Pay the $100 Registration fee
- Pay the $180 Security fee
- Pay the 1st of 10 payments of Hebrew School tuition, or
- Pay tuition in full
- Create payment plan with Office at info@downtownjewish.com

- I authorize all medical and surgical treatment, X-ray, laboratory, anesthesia, and other medical and/or hospital procedures as may be performed or prescribed by the attending physician and/or paramedics for my child and waive my right to informed consent of treatment. This waiver applies only in the event that neither parent/guardian can be reached in the case of an emergency.

Tuition Information
Hebrew School & Bat-Mitzvah Club: $100 Registration {non-refundable} $180 Security per student. (non-refundable) $1400 Tuition Bat-Mitzvah Club is an additional program to Hebrew School. $1200 per student {not including end of year ceremony}

Monthly Payment Agreement
- I understand I will make my first tuition payment wih the Registration Fee and Security fee after completing the Online Registration Form on the Payment Plan Form.
- I understand my card will be charged equal payments on the first of each month from Sept 1st/22- May 1st/23 for the remainder of my tuition, as per the tuition selections herein.
Registration & Security Fee Agreement*
- I understand I will make a payment after completing the Online Registration Form for the registration fee of $100 and the security guard fee of $180
- I understand the registration fees and security guard fees are non-refundable.
Payment Contract
- I understand that I am responsible to pay according to the above agreed upon payment schedule.
- If sufficient funds are not available or if the account has been closed, I understand that my account will be charged $25 for each transaction that could not be processed.
- I agree that my child(ren) will not be allowed to attend classes unless tuition payments are current.
- I understand that my child(ren) are enrolled for the entire year. The school cannot issue refunds or credits for illness, holidays, or family vacations.
- If the school is closed due to or resulting from a weather emergency or other unforeseen circumstance, there will be no make-up days, refunds, or credits for days that school is not in session.
- In the event of early withdrawal, I understand that I am responsible for a pro-rated amount of the tuition that is added to a withdrawal fee equal to 30% of the annual tuition.
- The registration fees and security guard fees are non-refundable.

Parent Release for Media Usage*
- Unrestricted Usage: I give unrestricted permission for my child's image to be used in print, video, and digital media. I agree that these images may be used by DJCC for a variety of purposes and that these images may be used without notifying me further. I do understand that the child's last name will not be used in conjunction with any video or digital images.
Kids love to see themselves in photos and parents love to see the smiling faces and activities going on with their children while they are not around We take photos and videos each and every day at Hebrew for Kids, updating our photo page and allowing you a glimpse into our classrooms and all the fun we are having! We are very sensitive to your privacy. Please NOTE: The Hebrew school photo link can only be accessed on our Hebrew school page on our website – not from the usual home page. Our Facebook page is a private page and only open to invited Hebrew school families.
I give permission for the following people to pick up my child from HFK:
Trips & Nature Walk Permissions
- I give permission for my child(ren) to be taken off School premises for Educational related activities or trips provided by Downtown Hebrew for Kids.
- I understand that this may either be by foot or in a licensed insured vehicle chosen for a trip day or excursion.
During the course of the year, there may be occasions where the children will be visiting a nursing home, supermarket or fun zone. All families will be notified in advance. To help expedite the process and have permission slips on file please sign below.
