1. I give permission for childcare personnel to have access to my child’s records.
2. I give permission for my name, telephone number and e-mail to be included in the class list that will be distributed.
3. I authorize the faculty and staff of DJC to photograph or video my child for the purpose of social media content, website content, advertising, and other marketing purposes for no additional consideration.
4. I have read, completed, & returned the Swim Central Form & Distracted Adult form.

5. I give permission for my child(ren) to be taken off school premises for educational related activities or trips provided by Downtown Jewish Camp. I understand that this may either be by foot or in a licensed, insured vehicle chosen for a trip day or excursion.

6. I allow my child to participate in food-related activities such as baking and special occasions such as holiday or birthday parties wherein food is involved or provided
7. I give permission to DJC to administer treatment to my child in the event of an emergency if I cannot be reached or if the nature of the incident, as determined by school personnel, is such that emergency personnel must be contacted first. I give consent for my child be transported by ambulance if the situation warrants it and for my child to be treated at the chosen hospital
8. I understand that DJC is NOT an allergen-free facility and that if my child has any of these allergens or sensitivities, he or she may come in contact with allergenic foods or substances. This includes but is not limited to peanuts, tree nuts, gluten, fish, wheat, eggs, dairy, and soy food products, as well as latex products and environmental allergens.
9. Parents and DJC are working cooperatively to assure that children are provided with nutritious snacks and meals. I agree to provide a nutritious lunch and a mid-afternoon snack for my child if not signed up in the DJC lunch program.

10. Search Agreement: I understand that my child and I have no expectation of privacy in any item that is DJC-owned, including but not limited to any electronic devices or online services, and agree that DJC authorities have the right to conduct random inspections of any DJC-owned items to protect student and staff health, safety, and welfare. In addition, in the event the DJP has reasonable grounds to believe my child possesses an unapproved item or has used a personal or DJC-owned item in a manner that violates DJC policy or any laws or regulations, the DJC reserves the right to conduct a search of my child’s pockets, bags, outerwear, shoes, electronic devices, any other item belonging to or in the possession of my child, or any online platform, account, or otherwise my child may have accessed. I understand that unless the situation potentially constitutes an emergency, the DJC will first attempt to notify me if a search is necessary and will have two or more staff present for a search; however, my signature on this document constitutes my and my child’s consent to conduct random and for-cause searches without additional or further consent when deemed necessary by DJC officials. This consent extends to all DJC activities, either at or away from the DJC’s campus.

11. By signing below, I indicate that I have the understanding and capacity to make this agreement and that I am fully informed as to and understand the contents of this document. I UNDERSTAND AND HEREBY AGREE TO ASSUME ALL OF THE RISKS WHICH MAY BE ENCOUNTERED WITH ENROLLMENT OF MY CHILD AT DJC. I DO HEREBY AGREE TO INDEMNIFY AND HOLD HARMLESS DJP and its agents and employees, and their heirs and assigns (collectively, the “Releasees”), from any and all past, present, future known and unknown liabilities, actions, causes of actions, claims, expenses, and damages, INCLUDING THOSE CAUSED BY THE NEGLIGENCE OR FAULT OF THE RELEASEES, and including without limitation, interest, penalties, court costs, attorney’s fees, and expenses on account of injury to myself, my child, or my property which I now have or which may arise in the future in connection with my child’s enrollment at DJC which is not the result of gross negligence, intentional neglect, or willful or wanton conduct by the Releasees.

12. I agree that any dispute related to or arising out of this Agreement or my child’s enrollment at DJC will not be taken to court, except if required by law or to enforce an arbitration award. Disputes will first be addressed through good-faith discussions (up to 30 days) among themselves, then, if unresolved, through mediation with the National Center for Life and Liberty or another agreed mediator. If mediation fails, the Parties agree to take the matter to final, binding arbitration, and if no arbitrator is agreed upon, to the American Arbitration Association. The arbitrator’s decision will be final, and I EXPRESSLY WAIVE THE RIGHT TO SUE THE RELEASEES IN CIVIL COURT EXCEPT AS ALLOWED ABOVE.

13. I further state that I have carefully read the foregoing information and know the contents thereof, and I sign this document as my own free act. I understand and agree that this Enrollment Agreement, together with the completed Registration Form, constitute a binding legal agreement between me and DJP. If any provision is found unenforceable, the remaining provisions shall continue in full force and effect.
