1. I give permission for childcare personnel to have access to my child’s records.
2. I give permission for my name, telephone number and e-mail to be included in the class list that will be distributed.
3. I authorize the faculty and staff of DJP to photograph or video my child for the purpose of social media content, website content, advertising, and other marketing purposes for no additional consideration.
4. I have read, completed, & returned the Swim Central Form & Distracted Adult form.

5. I give permission for my child(ren) to be taken off school premises for educational related activities or trips provided by Downtown Jewish Preschool. I understand that this may either be by foot or in a licensed, insured vehicle chosen for a trip day or excursion.

6. I allow my child to participate in food-related activities such as baking and special occasions such as holiday or birthday parties wherein food is involved or provided
7. I give permission to DJP to administer treatment to my child in the event of an emergency if I cannot be reached or if the nature of the incident, as determined by school personnel, is such that emergency personnel must be contacted first. I give consent for my child be transported by ambulance if the situation warrants it and for my child to be treated at the chosen hospital
8. I understand that DJP is NOT an allergen-free facility and that if my child has any of these allergens or sensitivities, he or she may come in contact with allergenic foods or substances. This includes but is not limited to peanuts, tree nuts, gluten, fish, wheat, eggs, dairy, and soy food products, as well as latex products and environmental allergens.
9. Parents and DJP are working cooperatively to assure that children are provided with nutritious snacks and meals. I agree to provide a nutritious lunch and a mid-afternoon snack for my child if not signed up in the DJP lunch program.

10. Search Agreement: I understand that my child and I have no expectation of privacy in any item that is DJP-owned, including but not limited to any electronic devices or online services, and agree that DJP authorities have the right to conduct random inspections of any DJP-owned items to protect student and staff health, safety, and welfare. In addition, in the event the DJP has reasonable grounds to believe my child possesses an unapproved item or has used a personal or DJP-owned item in a manner that violates DJP policy or any laws or regulations, the DJP reserves the right to conduct a search of my child’s pockets, bags, outerwear, shoes, electronic devices, any other item belonging to or in the possession of my child, or any online platform, account, or otherwise my child may have accessed. I understand that unless the situation potentially constitutes an emergency, the DJP will first attempt to notify me if a search is necessary and will have two or more staff present for a search; however, my signature on this document constitutes my and my child’s consent to conduct random and for-cause searches without additional or further consent when deemed necessary by DJP officials. This consent extends to all DJP activities, either at or away from the DJP’s campus.
